Develop public awareness of health care needs of our community.
Utilize existing community resources to their fullest capabilities.
Promote appropriate use of hospital emergency departments.

The Home and Community Based Services Program is designed to serve people in the community who would otherwise require nursing home care.
Services are prescribed and arranged according to the individual needs of the recipient. A tailored plan of care is developed by the Case Management Team in conjunction with the recipient and the attending physician.
This case management service is also available to private pay individuals.
